Slc Jewel Box.
This is not a species, it is a hybrid. See and for quick discussions of orchid names.
Dear Azalea,Is there a clonal name associated with this particular plant of yours? (There are several Jewel Box clones out there in the commercial market)

Sorry - This is not my plant- I saw it on a visit to a GH in Florida at the first Fl Swap about 3 years ago. It was such a beauty, just had to snap it.
