My guess is that the plant needs something. I was going to fertilize but what's the best type?

First you need to use a good fungicide on them. Don't try to fertilize a possibly sick plant.
Do you have leaves that are dying and falling off? You need a fertilizer for evergreens.

Hi PWYLL, dont really know your temp, but the box could just be suffering from the dry heat, also, how old is the box and when was it planted, box needs a rich well manured soil as it will be a plant that is in situ for a very long time, it is also constantly growing, even the dwarf box needs feeding, watering and a rich soil to stop the roots from dehydrating in the heat, so any extra info would be a help. box can also be attacted by various deseases so any info is a must to help you out.
